Some extensions, even though not in use, may load associated scripts (such as Javascript) etc in anticipation of their use which could slow things down. Whether that's a noticeable performance drop is another matter entirely.
There are many factors affecting site loading times and performance, not the least of which is your hosting infrastructure, and internet connection (not to mention your site visitors' internet service(s)).
My general advice is to use extensions sparingly. Use only what you need. Joomla is pretty versatile, lots of stuff can be done out of the box.
On shared hosting, concerning the performance of websites, generally, you will have other factors to deal with since you're sharing server resources with dozens, perhaps hundreds of other websites.
On a VPS hosting plan, that wouldn't be so much of an issue, since you have more dedicated resources for *your* own website(s).
For avoiding security risks I would stick with the reputable extensions found on the JED but I don't know that there is any guarantee that any extension is 100% "safe" from security issues.
